Emergency Room Enhancement Case Manager

Mark Twain Behavioral Health is seeking a full-time Emergency Room Enhancement Case Manager for our Kirksville Missouri service area.  This clinical position provides coordination of community-based services in a variety of settings to maximize independent functioning of clients and promote recovery and self-determination.  

 

Role Responsibilities

  • Develop and maintain positive relationships with hospital staff, ambulatory personnel, law enforcement and other collaborating agencies
  • Perform on-site mental health screenings/assessments in the community
  • Provide clinical crisis responses for clients with psychiatric, substance use and co-occurring disorders
  • Provide extensive case management and care coordination
  • Coordinate service delivery with appropriate agencies and monitor client participation
  • Complete program specific documentation
  • Arrange and participate in multi-disciplinary team meetings on behalf of assigned clients
  • Rotate the Access Crisis Intervention (ACI) on-call mobile response with other qualified mental health professionals
  • Maintain consistent updated communication with the Emergency Room Enhancement Coordinator for coordination of care
  • Guide treatment development with consumer and other stakeholders and maintain clinical record according to standards
  • Represent MTBH in the community by attendance at local interagency teams as assigned
  • Maintain a professional code of ethics at all times
  • Behave in professional and courteous manner with co-workers, supervisory staff, community members, other agency staff, and consumers of behavioral health services and their families
  • Serve as client advocate and adhere to all client rights including confidentiality
  • Complete daily schedule in a timely manner
  • Participate in Quality Improvement reviews, outpatient clinical staff meetings and in-service training programs
     

Adherence to the MTBH Ethical Code of Conduct and all policies is required.

 

Qualifications

  • Master's Degree in Counseling, Social Work or Psychology
  • Licensed in the state of Missouri or licensed-eligible
  • Minimum of three years of experience in working with persons in crisis, with clients and their families receiving psychiatric, substance use, or developmental disabilities services
  • Demonstrated knowledge of alcohol and drug addictions or certification as an alcohol and drug counselor
  • Excellent written and verbal communication skills
