Your MTBH Journey

Your Journey Starts Here

We are here to help.

The first step in beginning services with MTBH is to complete our intake process.  This can be completed by speaking with one of our intake coordinators over the phone or in person.

Please have available the following items for your intake:

  •   Your personal information (address, phone number, social security number)
  •   Driver’s License or State Identification Card
  •   Your Medicaid, Medicare, or other insurance cards
  •   A list of all medications you are currently taking
  •   If you have received any mental health or substance use services in the past, bring the name and contact information
  •   If you have been referred to us, the name of the referral source and contact information

If you don’t have the information listed above, please don’t let this stop you from contacting us. We will help you obtain what is needed.

Your Journey Begins

During your initial visit, either over the phone or in person, our intake coordinator will work with you to determine what brings you to MTBH. Once the intake is completed, they will connect you with the appropriate treatment provider and program.

Your Journey Continues

Based on your individual needs, we will work with you to design your plan of care.  As your journey is your own it will be specific to you only.

If for any reason you need to reschedule an appointment, please call the respective office location.
