Mark Twain Behavioral Health is seeking a full-time HELPING OTHERS PROVIDE ENCOURAGEMENT (HOPE) MULTIDISCIPLINARY TEAM (MDT) NAVIGATOR for our KIRKSVILLE, MISSOURI service region. The primary objective of the HOPE MDT Navigator is to provide support and coordination with cross-system staff to ensure that youth and family needs are met. The Navigator will monitor and drive referrals to services, appointments, meetings, treatment goals, etc., to ensure care is being coordinated and families are engaged. In addition, the navigator will connect with the Missouri Behavioral Health Council (MBHC) Department of Mental Health (DMH) and the Children’s Division (CD) to discuss opportunities for further coordination and success. HOPE MDT Navigators will meet quarterly with peers, MBHC, DMH, and CD for training and collaboration.  Navigators will leverage CareManager for monitoring and outcome tracking. 

 

Role Responsibilities

  • Provide intensive care coordination based on the ten (10) principles of the wraparound process

  • Establish ongoing relationships with multidisciplinary system partners and families

  • Coordinate and monitor services with local systems of care for children and youth with behavioral health needs

  • Attend monthly conference calls, webinars, and face-to-face meetings/trainings.

  • Attend all HOPE MDT Navigator meetings coordinated by the Missouri Behavioral Health Council (MBHC), Department of Mental Health (DMH), and the Children’s Division (CD)

  • Work closely with designees from MBHC, DMH/Division of Behavioral Health (DBH) and Division of Social Services (DSS), and CD

  • Complete reporting requirements

  • Assure all supporting documentation is present in each client file

  • Behave professionally and courteously with co-workers, supervisory staff, community members, as well as consumers of behavioral health services and their families.

  • Complete daily schedule in Carelogic and Bamboo

  • Adhere to all client rights, including confidentiality

     

Adherence to the MTBH Ethical Code of Conduct and all policies is required

A current driver's license is required.

 

Qualifications

Minimal requirement:

  • Bachelor’s degree in a human services field such as psychology, social work, counseling, or criminal justice 
  • Prefer at least two (2) years experience in the field of behavioral health and/or children's protective services

A HOPE MDT Navigator must be: 

  • Knowledgeable about the system of care continuum, including but not limited to the operation of the community providers, the crisis system, inpatient psychiatric/residential resources, and guardianship process (for transitional-age  youth);
  • Knowledgeable of trauma-informed principles to support youth and families recovering from trauma, violence, and/or adverse life experiences;
  • Knowledgeable about behavioral health disorders, especially within the child and youth population;
  • Proficient in public speaking and experienced in delivering professional training.
